Post Discharge Application

The aim of the project was to develop a post discharge application. This enables practitioners to extract the information necessary for patient to know and show them on their smartphone. The other goal was to improve efficiency and effectiveness of communicated information leading to better recovery for patients. The project was funded by Orion Health New Zealand.
